Congenital patellar dislocation: a difficult diagnosis

Bimbo di 22 mesi, che era seguito in altra sede per lussazione congenita dell’anca e generica “contrattura in flessione” del ginocchio (c).

A 22-month-old boy was treated elsewhere for congenital hip dislocation and a generic “flexion contracture” of the knee (c).

At OrthoChildren center, following a thorough clinical evaluation, a suspicion of congenital patellar dislocation was raised. Ultrasound (a) confirmed the diagnosis, highlighting the lateral dislocation of the patella (*) with respect to the femoral trochlea (arrows): at this age, in fact, the patella is not ossified, therefore it is not visible on simple x-rays.

The surgical procedure performed by Dr. Lampasi allowed the centering of the patella to be restored, which is clearly visible on x-rays taken several years later (b). The clinical picture has significantly improved, and full extension and correct knee function have been maintained over time (d).

Congenital patellar dislocation is a rare and little-known condition. Diagnosis and treatment require a great deal of experience.
